Insurance Waivers for Players & Staff

All athletes using the TeamLVC Training Center will be asked to fill out JVA waivers before attending a practice, lesson, open gym or tournament.

All non travel team member lesson participants must fill out both the League Apps registration as well as the JVA medical form/waiver.
